Blood glucose is measured, with the 1st drop of blood or the 2nd drop of blood? As long as you ask the doctor and nurse, you usually get two answers: some say the first drop of blood, and some say the second drop of blood.
So, which is more accurate, or is it all right?

1. Blood Glucose Monitoring Guidelines 2015 Edition
The "Guidelines for the Clinical Application of Blood Glucose Monitoring in China (2015 Edition)" regulates the following before measuring blood glucose:
(1) Clean the blood collection site (such as the side of the finger abdomen), wash the hands (especially the blood collection site) with soap and warm water, and dry them with a clean napkin or cotton ball;
(2) After cleaning, the arm where the blood collection site is located will naturally droop for a while, then massage the blood collection site and use the appropriate blood collection device to obtain a sufficient amount of blood samples, do not squeeze the blood collection site to obtain blood samples, otherwise the inter-tissue fluid will dilute the blood samples and interfere with the blood glucose test results.
Although the guidelines do not specify the 1st or 2nd blood, from the description, there is no special emphasis on the use of the 2nd blood, and the blood can be taken directly when it is ready by default, which is the 1st blood we understand.
2. 2016 "Standardized Use and Treatment and Quality Management of Portable Blood Glucose Meters"
Code of practice
(1) Select 75% ethanol to disinfect the blood collection site, and do not choose other disinfectants that are interfering with the detection, such as iodophor.
(2) After the ethanol has dried, blood collection can be carried out.
(3) After the blood collection needle punctures the skin, gently press the blood to flow out naturally, wipe the first drop with a disinfectant cotton ball, and then drop the second drop of blood into the designated area on the test strip area. Do not squeeze too hard after puncturing the skin, so as not to interfere with the results with the tissue fluid.
(4) Record the inspection results.
From the perspective of the release year, the two documents were published one after the other, the first was published in 15 years, and the second was published in 16 years.
From the perspective of the issuing institutions, the first guideline was issued by the Diabetes Branch of the Chinese Medical Association, and the second specification was issued by the Laboratory Medicine Branch of the Chinese Medical Association and the Clinical Laboratory Center of the National Health Commission.
Based on the above two points, the provisions of the 2016 "Standardized Use and Treatment and Quality Management of Portable Blood Glucose Meters" are relatively standardized, and it is recommended to use the second drop of blood when measuring blood glucose.
