"Life and Death Speed" - CCU rescue scenario simulation actual combat

author:Tsinghua First Affiliated Hospital

In today's increasing public health awareness and medical health needs, it is increasingly important to strengthen the theoretical learning and operational skills of nursing staff. In order to innovate and practice the teaching system and improve the quality of talent training, the CCU of Beijing Huaxin Hospital (the first affiliated hospital of Tsinghua University) conducted a simulated practical assessment of the rescue scenario of "life and death speed".

"In the process of inspecting the ward, 10 patients were found to be unconscious, and they were quickly rescued!" Wang Lining, the assessment officer and head nurse of the CCU, immediately issued a task order. "What's wrong with you? Wake up! Wake up! Two nurses quickly arrived at the bedside, one performed CPR for the patient, and one person quickly established two venous pathways, calm and self-assured, division of labor and cooperation: "The patient loses consciousness, judges that the carotid artery pulse disappears, and quickly notify the doctor!" Wang Lining accurately and quickly issued oral medical instructions, and the nurse repeated the medical instructions while skillfully executing: "Epinephrine 1mg static push, dopamine 20mg static push, 10ug/kg/min pumping, m-hydroxylamine 2mg/h pumping, sodium bicarbonate 100ml static ..."

"The patient suddenly convulses, the ECG monitor shows ventricular fibrillation, 200J two-way wave defibrillation!" Under the order of Wang Lining, the prepared defibrillator was immediately put into use. "Prepare for endotracheal intubation" "Prepare to suck"... In the battle with the "god of death", the nurses accurately rescued and cooperated perfectly with the doctors, and finally turned the "patient" into a safe situation.

As a new nurse who has just been employed for a month, I also participated in this rescue scenario simulation combat assessment, compared to the calm and composure of senior nurses, skillful movements, I reflected on my lack of clinical experience, lack of clinical judgment and emergency response ability, easy to be busy in the process of emergency first aid, inaccurate judgment, etc., there are still many areas that need to be improved. After the assessment, Nurse Wang Lining also pointed out the problems that the nurses had in the simulation one by one, and analyzed and summarized with everyone to help us improve continuously.

Rescue scenario simulations can be repeated over and over again, but there is no drill in life, and medical staff must always be prepared. I remember a sentence that my teacher said to me when I first entered the department: "Health is related to life." Our profession is a conscience job! ”。 This requires us to always be rigorous and serious in our work, and the drug dosage is the most basic, and we must calmly respond to unexpected situations, keep our minds clear, and be responsible for each patient.

"Wenxin Carved Dragon" said: "Practice a thousand songs and then xiaosheng, look at a thousand swords and then recognize the instrument." This means that after mastering many pieces of music, you can understand music, and after observing many swords, you know how to identify the sword. Nursing work is also the same, take every practical assessment seriously, constantly exercise their operational skills and mentality, improve their clinical judgment and emergency response ability, in order to win a glimmer of life for patients in the race against time, in order to do a good job of this "conscience work"!
