Why is TCM not divided into disciplines? Why is good TCM a general practice?

Foreword Referring to the division of Traditional Chinese medicine in Western Law, it is divided into Traditional Chinese Medicine, Traditional Chinese Medicine, acupuncture, and is divided into gynecological, infant, internal, external, skin and other departments, which is a living division and dismemberment of Chinese medicine. A truly good Chinese medicine practitioner must grasp the human body as a whole and identify the condition from the following table throughout the body.

There is a joke that you may have heard:

One patient had a toothache and went to the dentistry department for a tooth extraction, because the doctor's hand was shaking, and the extracted tooth was not stable and fell into the throat. The doctor instructs the patient to go to the ear, nose and throat department quickly. The patient ran a little urgently, and when he reached the laryngology department, the teeth had slipped down to the stomach after an examination. The patient ran to the gastroenterology department again, a gastroscopy, the fallen teeth were no longer in the stomach... Finally, the patient pulled his pants and cocked his ass in the proctology department, and the anorectal doctor pulled open a check and exclaimed: Oh my God, how did you grow a tooth in your anus? Hurry up and go to the dentistry to extract your teeth!

This is, of course, just a joke. But it can more or less reflect some of the practical drawbacks of medical sub-disciplines. Many patients go to the hospital and often don't know what department to hang. For Chinese medicine, from the beginning of college education, it is divided into Chinese medicine, Chinese medicine, acupuncture, tuina and so on. Among them, TCM clinical is subdivided into pediatrics, gynecology, internal medicine, surgery, etc., and even more finely divided...

Although the call for "Chinese medicine in the folk" has become higher and higher in recent years, we also have to admit that many folk Chinese medicines are just "one trick to rule the world". For example, relying on one or two secret recipes, it is special to treat skin diseases, headaches, gynecological fibroids... Is this way of thinking true TCM?

01

Disadvantages of TCM sub-disciplines

I am sometimes asked what is the main treatment? I guess the other party is probably asking me which subject to focus on? I have always believed that the division of disciplines is a living separation and dismemberment of Chinese medicine. The originally integrated medicine, medicine and acupuncture are unreasonably forced apart to understand. Is it appropriate for TCM clinical to be based on the internal, external, gynecological, juvenile, dermatological and other disciplines of Western medicine?

Is there a dermatology department?

Many of the root causes of sores are in the internal medicine family. For example, thrush, sometimes the cause is intestinal carbuncles, that is, intestinal sores. Just applying plaster to the surface of the skin will not be good for a lifetime. Mr. Hu Xishu has a medical case: the appendage coix seed defeat sauce is scattered, the internal treatment is removed from the intestinal carbuncle, and the thrush on the muscle surface is naturally better.

Set up spiritual materials?

For mental illness, Western medicine focuses on the brain anyway, and all use central nervous system inhibitory drugs such as tranquilizer and chlorazapine. Jingfang Chinese medicine does not do this. The so-called "psychosis" is often rooted outside the brain. For example, when the soup (pill) is used to treat the "mental illness" caused by blood stasis in the lower abdomen

"Those who are mad, scorched with heat, less abdomen when whipped, those who urinate and self-interest, blood is healed."

Some "mental illness" is even just because the stool is blocked, such as the brain symptoms such as the crazy slurry of Yangming Dacheng Soup, and you have to start from the gastroenterology department to get through the stool.

Is there an oncology department?

The treatment of cancer in Chinese medicine is not as simple as using so-called anti-cancer drugs such as half lotus and snake tongue grass. Of course, it should not be copied to talk about cancer color becoming impossible to start. For doctors who have learned the "Treatise on Typhoid Fever and Miscellaneous Diseases", there is no essential difference in methodology between treating cancer and treating colds. For example, we have used Da Chai Hu Tang to treat rectal cancer more than once, and used Chai Hu Agent and Yin Chen Wu Ling To disperse the Chinese medicine to treat liver cirrhosis.

Is there a TCM Aesthetic Department?

Do not study internal medicine and wet water stasis and other miscellaneous diseases, freckle removal, beauty, weight loss can be effective? Effective is also temporary. Set up an infertility specialist? The entire human body does not recover, the uterine cold is not warm, the blood stasis is not removed, the dripping is not solved, the fibroids are not eliminated, the liver depression is not opened, the qi and blood are not restored, how to get pregnant... I won't give you any examples.

02

The origin of the sub-discipline of Chinese medicine

The division of Chinese medicine into disciplines is actually a matter of decades. Famous doctors of all generations, such as the doctor Zhang Zhongjing and the medicine king Sun Simiao, are not specialist doctors who only treat a certain type of disease. Zhongjing's "On Typhoid Fever and Miscellaneous Diseases" and Sun Simiao's "Qianjin Fang" are almost all incurable. Since ancient times, Chinese medicine is also usually not divided into medicine, their own prescriptions, their own preparations, famous doctors are usually proficient in pharmacy, typical such as Li Shizhen, who wrote the Chinese medicine work "Compendium of Materia Medica", there are also medical books "Qijing Eight Pulse Examination" and "Pinhu Pulse Science" passed down. Wang Ang, a famous physician in the Qing Dynasty, also wrote the classic medicine book "Materia Medica Preparation". At the same time, in ancient times, when doctors were diagnosed and treated, acupuncture and prescriptions were often used for each other.

Even during the Republic of China period, Chinese medicine was not divided into disciplines. Most of the TCM practitioners in the Republic of China practice medicine in the form of private clinics. The Chinese medicine practitioners we are familiar with, such as Hu Xishu, Zhu Weiju, Yun Tieqiao, Cao Yingfu, etc., all opened private clinics in those years. Traditional TCM training is basically through mentoring and discipleship.

So, why is the integrity of TCM broken?

This is because after 1949, the lack of medical treatment and medicine in China was serious, and the government vigorously developed traditional Chinese medicine, more accurately, vigorously developed the combination of Chinese and Western. However, the traditional TCM training method has a long cycle and obviously cannot solve the urgent need. As a result, a number of traditional Chinese medicine colleges have been established successively, and with reference to the Western medical education model, there are separate disciplines. In this way, the original TCM and TCM are separated, and the TCM clinic is subdivided according to each department. The current examination for practicing physicians is even more legally, completely separating prescriptions, acupuncture and Chinese medicine. Practicing acupuncturists do not have the right to prescribe, and practicing TCM practitioners cannot use acupuncture. Practicing Chinese medicine pharmacists, can not engage in treatment. In this way, in everyone's concept, Chinese medicine, like Western medicine, is also strictly divided into disciplines. Even when I go to the Chinese medicine hospital, I consciously hang up the dermatology, surgery, internal medicine...

Various "standards", often the same "book"

Everyone knows that Chinese medicine emphasizes the treatment of diseases and cures the root causes. However, when doctors are accustomed to the pattern of division, they often involuntarily divide diseases into different departments. This undoubtedly creates a kind of limited thinking. It is easy to become the symptom treatment idea of "headache medicine, foot pain medicine". And many times, different diseases, different symptoms, in fact, there is only one reason.

For example, the water in the body is wet, and you can be dizzy when you go to the head, such as Linggui Shu Ganzheng's "from the beginning of the dizziness, as a vibration shake", or as the "zhenzhen desire to sacral land" of the zhenwu tang certificate;

Wet water (water drinking) seepage on the scorch may cough, such as the small green dragon soup certificate;

Under the wet obstruction of the heart, may vomit, may not be hungry;

Water between the intestines, may leak, may urinate unfavorably;

Under the stagnation of water, the feet may be swollen, and it may cause urinary diseases mentioned by Western medicine, such as Wuling Scatter Certificate and Pig Lily Soup Certificate;

Water wetting into the meridian joints can cause cold pain in the joints, which is often referred to as rheumatism;

Water is wet and fills the muscles, and will be heavy;

Water and wet skin will swell, may also grow eczema and other skin diseases;

Wet water can also cause insomnia...

That is to say, the same book, water wetness, can cause different standards, the same cause leads to different diseases, triggers various symptoms, and involves various departments.

Therefore, a really good Chinese medicine must grasp the human body as a whole and identify the condition from the following table throughout the body. It cannot be divided into sections, nor can it be divided into sections. Otherwise, what is the difference with a blind man? And the person who can grasp the overall order of human body operation is naturally a general practitioner.
