The key to the Easter egg is whether the player obtains the Dark Shepherd Dagger in the Return of the Legion version and completes the quest "He's Watching You" (Purifying the Eye of Enzoth) in the Azeroth version of the Quest for Azeroth.

Saratas, Blade of the Dark Empire is an ancient dagger, forged from the claws of the Elder Gods, and specializes in whispering demagogy. Eventually, Saratas gained his own will, transfiguring into a human form, using void energy and psychic magic to bend reality for his evil ends. In the Conquest of Azeroth version, the player encounters Saratas before Neolosa. In this episode, the player is "given" the Eyes of Enzos, which appear permanently on the forehead, allowing us to see other fallen characters and occasionally special interactions with NPCs. After the quest is completed, the Eye of Enzos is purified and the effect disappears, but the player is given a "Twitching Eyeball" toy that can reproduce the effect.
Regarding Saratas, her villain's path was not completely ended in 11.0, according to executive creative director Chris · Mason, unlike previous villains, Saratas will not be active in just one expansion, players will face off against it throughout the Star Soul trilogy, and there may be more Dark Pastoral Easter eggs.
