The following picture is not unfamiliar. This was a sensational collapse accident in Linfen, Shanxi Province last year, which caused seven deaths and was reported by CCTV news and other mainstream media, with a great social impact.
Recently, the Emergency Management Bureau of Linfen City, Shanxi Province released an investigation report on the "11.24" large collapse accident. Recommendations were made to the persons and units responsible for the accident, of which 9 were criminally detained, including a 61-year-old supervisor:
1. Zhang Yonghong, male, 44 years old, member of the Communist Party of China, deputy manager of the metal structure branch of Chinalco International (Tianjin) Company (construction general contractor), project manager in charge of the project, has been away from work for a long time in the construction management process of the project. On December 28, 2023, he was criminally detained by the Anze County Public Security Bureau in accordance with the law.
2. Sun Limin, male, 38 years old, is the constructor (deputy project manager) of Chinalco International (Tianjin) Company (construction general contractor) in charge of the project. On December 20, 2023, he was criminally detained by the Anze County Public Security Bureau in accordance with the law.
3. Guo Mingfei, male, 30 years old, technical leader of the project of Chinalco International (Tianjin) Company (construction general contractor). On December 21, 2023, he was criminally detained by the Anze County Public Security Bureau in accordance with the law.
4. Zhang Lanji, male, 41 years old, legal representative of Shaanxi Zhiyongquan Industrial Co., Ltd. (subcontractor). On December 22, 2023, he was criminally detained by the Anze County Public Security Bureau in accordance with the law.
5. Huang Kaichun, male, 50 years old, the on-site person in charge of the contracted project of Shaanxi Zhiyongquan Industrial Co., Ltd. (subcontractor). On December 20, 2023, he was criminally detained by the Anze County Public Security Bureau in accordance with the law.
6. Ke Zengqun, male, 34 years old, person in charge of the raw material coal shed project, safety supervisor and technical instructor in the contracted project of Shaanxi Zhiyongquan Industrial Co., Ltd. (subcontractor). On December 21, 2023, he was criminally detained by the Anze County Public Security Bureau in accordance with the law.
7. Zhang Jingchen, male, 47 years old, the supervision director of Shanxi Construction Supervision Co., Ltd. (supervision unit) in charge of the project, has been out of supervision for a long time. On December 28, 2023, he was criminally detained by the Anze County Public Security Bureau in accordance with the law.
8. Quartz, female, 54 years old, representative of the supervision director of Shanxi Construction Supervision Co., Ltd. (supervision unit) in charge of the project. On December 26, 2023, he was criminally detained by the Anze County Public Security Bureau in accordance with the law.
9. Cui Bin, male, 61 years old, full-time supervisor (civil engineering supervision engineer) of Shanxi Construction Supervision Co., Ltd. (supervision unit) in charge of the project. On December 20, 2023, he was criminally detained by the Anze County Public Security Bureau in accordance with the law.
The key contents of the accident investigation report are as follows:
At 21:59 on November 24, 2023, Shanxi Yongxin Tonghai Railway Logistics Co., Ltd. Yongxin Railway Logistics Co., Ltd. Yongxin Railway Special Line Container Station Construction Project Coal Blending System Raw Material Coal Shed No. 2 Head Room During the pouring of concrete, a formwork support system collapsed accident, resulting in 7 deaths and direct economic losses of about 19.4671 million yuan.
The No. 2 machine head room covers an area of 63m2, 9m long, 7m wide, 36.14m high, the structural form is a reinforced concrete frame structure, 2 floors above ground, set up with four reinforced concrete frame columns, surrounded by connecting beams (no plates) at a height of 9.94m and 19.74m, and reinforced concrete beams and slabs at 29.74m and 36.14m. At 29.74m, the plate thickness is 120mm, and the longitudinal and transverse beam specifications are 500×850mm, 400×900mm, 300×850mm, 300×700mm, etc., and there is a 500×850mm, 500×800mm cantilever corbel beam on the north and south sides, and the concrete strength grade is C30. At the time of the accident, concrete pouring of beams and slabs at 29.74m was underway. The height of the formwork support frame is 29.74m, which belongs to the dangerous sub-project of more than a certain scale. The scaffolding body is erected by the scaffolding worker to 28.1m, and the carpenter erects the beam plate to support the scaffold from the 28.1m point upwards to the bottom of the beam slab (see Figure 1).
How the accident happened
At 20:00 on November 24, 2023, 4 workers from Shaanxi Zhiyongquan Industrial Co., Ltd. (subcontractor) and 1 pump truck operator from Anze Xusheng Commercial Concrete Co., Ltd. (commercial concrete manufacturer) went to the 29.74m of the No. 2 head room to pour the beam and slab concrete, and started pouring at 20:10, and at 20:46, 2 workers from Shaanxi Zhiyongquan Industrial Co., Ltd. went up to the 29.74m beam and slab to implement the concrete closing and film cover operation (see Figure 2).
At 21:59, when the pouring was almost completed, the corbel beam support frame on the south side of the No. 2 nose room was first tilted outward and deformed (see Figs. 3 and 4).
The formwork support system collapsed (see Fig. 5 and 6), and seven personnel on the working floor fell from the 29.74 m high roof to the ground coal funnel and were buried by about 30 m3 of reinforced concrete (see Fig. 7 and 8).
Direct cause
The longitudinal and transverse spacing of the upright rods of the formwork support system is too large, and the axial stress of the upright rods seriously exceeds the standard design value of the steel pipe (1). The step distance of the horizontal rod of the formwork support system is too large (1), the frame body is not provided with a horizontal scissor brace (2) (see Fig. 9), the height and width ratio of the frame body does not meet the requirements of the specification and the connecting wall piece (3) is not arranged at the full height (see Fig. 10), and the frame body and the frame structure are not effectively connected, and the overall stability of the scaffold is reduced. The above-mentioned unfavorable factors lead to the collapse of the formwork support system during the pouring of concrete.

The main problems that exist with regard to the responsible units
1. Chinalco International (Tianjin) Company (construction general contractor)
(1) Chaos in project organization and management
There are 13 project management personnel in the bidding documents, but there are only 4 in fact. Zhang Yonghong, the project manager, has been absent from his post for a long time. After the safety officer Li Linfeng was transferred from the project department on August 21, 2023, there was no full-time safety officer at the time of the accident (2). The technical person in charge, Guo Mingfei, did not perform his duties on the job. Failure to implement the safety production responsibility system for all employees (3), and on-site personnel on duty failed to perform safety production responsibilities in accordance with the provisions of the system.
(2) The safety management of critical projects is out of control

(4) Poor supervision of the project department
2. Shaanxi Zhiyongquan Industrial Co., Ltd. (subcontractor)
There are 11 management personnel, and the project manager is attached to the certificate, who has not actually performed his duties, and Huang Kaichun, the actual person in charge of the project department, does not meet the qualifications.
(1) Wang Kai, the project manager, did not arrive at his post to perform his duties.
(2) Failure to formulate a safety production responsibility system for all employees and various safety management systems (1).
(3) The special construction plan of the formwork support system was not prepared, it was not reviewed, and it was not reported to the general contractor Chinalco International (Tianjin) Company for approval, and the operator constructed with experience (2).
3. Shanxi Construction Supervision Co., Ltd. (Supervision Unit)
(1) Risk management of critical projects is out of control
(2) The supervisors do not perform their duties properly
4. Tonghai Railway Logistics Company (Construction Unit)
After the project is approved, the construction starts without going through the relevant formalities (4); Failure to effectively manage the construction unit and supervision unit; The safety inspection of the project construction is not in place.

After testing, the quality of the materials of the leased steel pipes, rotary fasteners and cross fasteners is lower than the national standard.
Determination of accident responsibility and suggestions for dealing with those responsible for the accident
In accordance with the relevant laws and regulations and the provisions of party discipline and government discipline, suggestions are put forward for the handling of the relevant responsible personnel and responsible units of the accident: it is recommended to transfer 9 people to the judicial organs, 26 public employees to be dealt with by party discipline and government discipline, and 5 accident-related enterprises and 11 relevant responsible personnel to be given administrative penalties for violations of laws and regulations.

(2) Suggestions on the handling of relevant corporate responsible personnel (14 people)
1. Chinalco International Engineering Co., Ltd. (state-owned enterprise) (3 people)

(2) Bi Xiaoge, male, 56 years old, member of the Communist Party of China, has served as deputy general manager and safety director of Chinalco International Engineering Co., Ltd. since September 2023. (3) Li Dongsheng, male, 52 years old, member of the Communist Party of China, has been the manager of the project management department of Chinalco International Engineering Co., Ltd. since July 2023. Suggestion: Aluminum Corporation of China should handle it in accordance with discipline and regulations.
2. Chinalco International (Tianjin) Company (state-owned enterprise) (7 people)
(4) Li Huaguo, male, 56 years old, member of the Communist Party of China, legal representative, secretary of the Party committee and chairman of Chinalco International (Tianjin) Company. Recommendation: In accordance with Article 95 of the Work Safety Law of the People's Republic of China, the Linfen Municipal Emergency Management Bureau shall impose a fine of 60% of the annual income in 2022; It will be handled by Chinalco International Engineering Co., Ltd. in accordance with discipline and regulations.
(5) Zhang Zhongwei, male, 55 years old, member of the Communist Party of China, deputy secretary of the Party Committee and general manager of Chinalco International (Tianjin) Company. Recommendation: In accordance with Article 95 of the Work Safety Law of the People's Republic of China, the Linfen Municipal Emergency Management Bureau shall impose a fine of 60% of the annual income in 2022; It will be handled by Chinalco International Engineering Co., Ltd. in accordance with discipline and regulations.
(6) Mao Zhenlong, male, 56 years old, member of the Communist Party of China, deputy general manager, safety director and chief engineer of Chinalco International (Tianjin) Company. (10) Zhang Minyi, male, 56 years old, member of the Communist Party of China, Secretary of the Party Committee and General Manager of the Metal Structure Branch of Chinalco International (Tianjin) Company. Recommendation: In accordance with Article 96 of the Work Safety Law of the People's Republic of China, the Linfen Municipal Emergency Management Bureau shall impose a fine of 30% of the annual income in 2022; It will be handled by Chinalco International Engineering Co., Ltd. in accordance with discipline and regulations.
3. Tonghai Railway Logistics Company (3 people)
(11) Chen Bo, male, 41 years old, member of the Communist Party of China, general manager of Tonghai Railway Logistics Company. Recommendation: In accordance with Article 95 of the Work Safety Law of the People's Republic of China, the Linfen Municipal Emergency Management Bureau shall impose a fine of 60% of the annual income in 2022.
(12) Xun Guowei, male, 45 years old, masses, deputy general manager of Tonghai Railway Logistics Company. Recommendation: In accordance with Article 96 of the Work Safety Law of the People's Republic of China, the Linfen Municipal Emergency Management Bureau shall impose a fine of 30% of the annual income in 2022.
(13) Chen Jie, male, 39 years old, member of the Communist Party of China, director of the station engineering department of Tonghai Railway Logistics Company. Recommendation: In accordance with Article 96 of the Work Safety Law of the People's Republic of China, the Linfen Municipal Emergency Management Bureau shall impose a fine of 30% of the annual income in 2022.
4. Shaanxi Zhiyongquan Industrial Co., Ltd. (1 person)
(14) Wang Kai, male, 36 years old, second-level constructor. The project leader in the "Professional Subcontract for Construction of Construction Engineering" provided by Shaanxi Zhiyongquan Industrial Co., Ltd., a professional subcontractor, provided the qualification of the second-level construction engineer to Shaanxi Zhiyong Company through an intermediary agency and obtained the corresponding remuneration, but did not participate in any actual work of the accident project. Recommendation: Anze County Housing and Urban-Rural Development Bureau shall punish according to law; The letter informs the Shaanxi Provincial Department of Housing and Urban-Rural Development (registration authority) to deal with its qualification certificate in accordance with relevant regulations.

(19) Zhang Jianfu, male, 54 years old, member of the Communist Party of China, principal of Anze County No. 1 Middle School, deputy secretary of the Party Working Committee and executive deputy director of the Management Committee of Anze Economic and Technological Development Zone from September 2019 to October 2023.
(20) Pei Wensheng, male, 41 years old, member of the Communist Party of China, deputy director of the Comprehensive Work Department of the Management Committee of Anze Economic and Technological Development Zone (presiding over the work).
(21) Xu Yonghui, male, 44 years old, member of the Communist Party of China, Secretary of the Party Committee of Tangcheng Town, Anze County.
(22) Cao Bin, male, 40 years old, member of the Communist Party of China, mayor of the People's Government of Tangcheng Town, Anze County.
(23) Shen Guangyao, male, 42 years old, member of the Communist Party of China, member of the Party Committee of the People's Government of Tangcheng Town, Anze County, and chairman of the National People's Congress.
(24) Wang Xiaofang, male, 47 years old, member of the Communist Party of China, deputy mayor of the People's Government of Tangcheng Town, Anze County.
(25) Gu Faming, male, 52 years old, member of the Communist Party of China, director of the Safety Supervision Office of Tangcheng Town, Anze County.
(26) Liu Feng, male, 46 years old, member of the Communist Party of China, deputy section-level cadre of Anze County Development and Reform Bureau.
(27) Zhao Genliang, male, 46 years old, masses, clerk of Anze County Development and Reform Bureau.
(28) Lei Jun, male, 43 years old, member of the Communist Party of China, director of the Housing and Urban-Rural Development Administration Bureau of Anze County.
(29) Han Yiqing, male, 49 years old, member of the Communist Party of China, senior cadre of Anze County Housing and Urban-Rural Development Administration.
(30) Meng Xiuhui, female, 46 years old, member of the Communist Party of China, head of the construction industry of Anze County Housing and Urban-Rural Development Administration.
The above 16 people are recommended to be dealt with by the Commission for Discipline Inspection and Supervision in accordance with laws and regulations according to the cadre management authority, and the results of the handling are notified to the Linfen Emergency Management Bureau.
(4) Recommendations for administrative punishment of the unit responsible for the accident
1. In accordance with the provisions of Article 114, Paragraph 1, Item 2 of the "Work Safety Law of the People's Republic of China", the Linfen Emergency Management Bureau shall punish Chinalco International (Tianjin) Company, Shaanxi Zhiyongquan Industrial Co., Ltd., Shanxi Construction Supervision Co., Ltd., and Tonghai Railway Logistics Company according to law.
2. Write to the Changzhi Municipal Market Supervision and Administration Bureau to punish the Jincheng Construction Erection Equipment Leasing Station of Changzhi High-tech Zone according to law.
(5) Suggestions for handling relevant units
1. The Management Committee of Anze Economic and Technological Development Zone, the Party Committee and Government of Tangcheng Town, Anze County, the Housing and Urban-Rural Development Administration of Anze County, and the Development and Reform Bureau of Anze County have made in-depth written inspections to the Anze County Party Committee and County Government.
2. The Anze County Party Committee and the County Government made an in-depth written inspection to the Linfen Municipal Party Committee and the Municipal Government.
3. The Linfen Municipal Safety Production Committee interviewed the Anze County Party Committee and the county government.
