A few days ago, the Shanxi Bureau of the State Mine Safety Supervision Bureau released the investigation report on the "1.14" general mechanical and electrical accident of the Majiliang Mine of Shanxi Datong Jinneng Holding Coal Industry Group Co., Ltd.
The report pointed out that mining companies have problems such as refusal to implement regulatory inspection instructions and illegal cross-border mining. The accident investigation team recommended that the 13 relevant responsible persons be held accountable, among them, the mine manager Yao Moumou and two others were transferred to the judicial authorities for handling in accordance with the law.
At 22:28 on January 14, 2024, a general mechanical and electrical accident occurred at the 8202 fully mechanized mining face in the second panel area of the Carboniferous 3# coal seam under the Majiliang Mine of Jinneng Holding Coal Group Co., Ltd., resulting in 1 person killed, 1 person seriously injured, and a direct economic loss of 2,117,361 yuan. After the accident, the mine reported it late. In accordance with relevant laws and regulations, the Shanxi Bureau took the lead in organizing the accident investigation.
The direct cause of the accident
The investigation found that Majiliang Mine refused to implement the supervision and supervision instructions, illegally arranged the 8202 fully mechanized mining face outside the mine boundary and illegally organized mining, and organized personnel to take risks without taking anti-falling measures and setting up safety alerts when they knew that there were risks, resulting in the unstable dumping of the gantry support and the accident.
Indirect causes of the accident
(1) Majiliang Mine organizes production in violation of laws and regulations. refusing to comply with the instructions of the State Mine Safety Supervision Bureau requiring the mine to return to the permitted mine boundary, and organizing the unsealing of the working face arranged in the blank area beyond the boundary without authorization; After the working face is unsealed, the team is blindly organized to enter the working face for operation without restoring the personnel positioning, monitoring system and video system; The deputy mine director of mechanical and electrical engineering, the deputy mine director of production and the chief engineer have been on leave for a long time, and the "five-post mine manager" is not fully equipped to organize production.
(2) There is a serious lack of safety management in Majiliang Mine. After the long-term closed working face was unsealed, neither risk identification, implementation of procedures and measures, nor inspection and maintenance of key equipment such as crawler transporters, and the organization of hidden danger investigation and operation training was also a mere formality; In the high-risk parallel operation of multiple teams, on-site safety management and coordination commanders were not arranged, and equipment installation-related operations were blindly carried out.
(3) The safety responsibilities of the Majiliang mine have been lost at all levels. The driver of the transport vehicle did not carefully check the air pressure of the vehicle and blindly operated the vehicle and the lifting platform; The maintenance operator did not carefully check whether the top beam of the portal bracket was completely placed into the card slot of the lifting platform; The cadres on duty did not require the on-site personnel to take corresponding measures immediately, that is, they acquiesced in the operators to leave the site, which eventually led to the loss of control of the risk of the portal bracket.
(4) The lessons learned from the accident were not in place. After a number of general mechanical and electrical accidents such as "5.14" in 2019, "4.13" in 2022 and "7.14" in 2023, Majiliang Mine failed to deeply learn the lessons of the accident and failed to take targeted control measures, resulting in the recurrence of the same type of accident.
(5) The superior company of Majiliang Mine failed to conscientiously implement its main responsibility. Jinneng Holding Coal Group Co., Ltd. failed to conscientiously implement the safety management responsibility of the Majiliang Mine, and the daily supervision and inspection of the mine was not strong, the quality was not high, and the coverage was incomplete, and the problems of cross-border mining and chaotic safety management were not discovered.
(6) The local coal mine safety supervision department did not strictly implement the supervision responsibility. The Datong Municipal Emergency Bureau has failed to implement the guidelines, policies and regulations of the higher authorities on safe production
Thirteen people were held accountable
The accident investigation team recommended that the 13 responsible persons be held accountable.
Yao Moumou, the director of the Majiliang Mine, was fully responsible for the work of mine safety production, and was the first person responsible for mine safety production, refused to implement the supervision and supervision instructions to stop cross-border production, arranged the mining face outside the mine boundary without authorization, and forced others to take risks in the case of safety monitoring, personnel positioning and video systems were not installed, and was directly responsible for the occurrence of the accident and transferred it to the judicial authorities for handling in accordance with the law.
Zhao, deputy captain of the Majiliang mine installation team, the person in charge of the on-site operation on duty at the accident, when he was operating in the maintenance of the portal support of the return air along the groove of the 8202 coal mining face, he knew that the top beam of the support was not connected to the top, and failed to take effective anti-falling measures according to the requirements of the operating procedures, and was directly responsible for the accident and transferred it to the judicial authorities for handling in accordance with the law.
In addition, the secretary of the party committee of Mazhiliang Mine was dismissed from his position within the party and given administrative punishment;
Two people, including the deputy mine director of safety and the deputy mine director of mechanical and electrical engineering, were given serious warnings, dismissals and administrative punishments within the party;
5 people, including the head of the mechanical and electrical management department of the Ma Jiliang mine, the captain of the installation team, and the secretary of the party branch of the installation team, were given party discipline and government sanctions;
2 people, including the deputy general manager in charge of mechanical and electrical work and the director of the mechanical and electrical management department of Jinneng Holding Coal Industry Group Co., Ltd., were given party disciplinary sanctions, organizational treatment or administrative punishments;
One member of the comprehensive administrative law enforcement detachment of the Datong City Emergency Management Bureau (Local Coal Mine Safety Supervision and Administration Bureau) was organized.
In view of the illegal act of over-layer cross-border mining of Majiliang Mine, it was transferred to the Datong Municipal Planning and Natural Resources Bureau for handling in accordance with the law; An administrative fine of 6.65 million yuan was imposed on the Majiliang Mine, and it was included in the list of seriously untrustworthy entities in production safety.
The person in charge of the Shanxi Bureau stressed that the life of the miner and the life of the mine manager are the same, both are supreme, and they are only once. The main person in charge of a coal mining enterprise must adhere to the supremacy of the people and the supremacy of life, firmly establish the concept that accidents can be prevented and controlled, know their responsibilities and fulfill their responsibilities, and ensure that they run, manage and perform their duties in accordance with the law. The principal responsible persons of coal mining enterprises who fail to perform their statutory duties in accordance with the law and cause accidents, especially those who do not learn the lessons of the accident and repeatedly repeat the mistakes of the past, shall be investigated and held accountable in accordance with the relevant provisions of the Criminal Law in accordance with Articles 134 and 135 of the Criminal Law, and those suspected of committing crimes must be transferred to the judicial organs for handling; The accident mine shall be subject to administrative punishment in strict accordance with the provisions of Articles 94 and 114 of the Work Safety Law of the People's Republic of China. At the same time, it is necessary to persist in double investigation in one case and investigate the responsibility of the superior company and its relevant departments.
