In the early morning of August 21, 2023, the Luzilin River Basin in Jinyang County, Liangshan Prefecture was affected by short-term heavy rainfall in the upper reaches of the flash flood and debris flow disaster, and the construction personnel of the steel processing plant of the JN1 Project Management Department of the G4216 Line Jinyang-Ningnan Section of the Expressway (hereinafter referred to as the JN1 Project Department) of the Sichuan Highway and Bridge Construction Group Co., Ltd., which is located in the lower reaches of the Ludulin River, were washed away, resulting in 6 deaths, 46 missing contacts and 21 injuries. A few days ago, the executive meeting of the Sichuan Provincial Government deliberated and approved the "8.21" flash flood disaster investigation and assessment report in Jinyang County, Liangshan Prefecture. After investigation, it was determined that the "8.21" flash flood disaster in Jinyang County, Liangshan Prefecture was a flash flood and debris flow disaster caused by multiple collapses, landslides, and debris flows caused by concentrated heavy rainfall in the upper reaches, and a large amount of sediment was carried by the flash flood, which washed away the construction personnel of the steel bar processing plant of the JN1 project department. Local party committees and governments and relevant departments have failed to perform their duties in disaster prevention and response, daily flood control management, and project construction supervision, and some cadres in Liangshan Prefecture and Jinyang County have acquiesced and connived at enterprises to lie and conceal the number of missing people.

The investigation team found that short-term heavy rainfall was an important factor that induced disasters; unfavorable topography, landform, and geological conditions aggravated the hazards of disasters; poor flood and sand transport in rivers aggravated the disaster effects; and illegal residents and failure to move people in time to avoid danger during the flood season were important reasons for the loss of contact with casualties.

The investigation team found that the relevant personnel of the Shudao Investment Group Co., Ltd. and its subordinate enterprises jointly organized and carried out false reporting and concealment; in order to cover up the act of making false reports and concealing information, the Shudao Investment Group Co., Ltd. organized funds in violation of regulations to deal with the aftermath, and the relevant personnel did not report their knowledge; and some cadres in Liangshan Prefecture and Jinyang County tacitly connived at the acts of lying and concealing the number of missing people. The JN1 project department was seriously lucky, did not organize the transfer of risk avoidance in time, and there was an obvious lack of implementation of the end responsibility, and the Jinyang County Party Committee, the county government and relevant departments and Lubi Town did not track and verify the implementation of the transfer and hedging. The JN1 project department illegally used temporary land, occupied the river in violation of laws and regulations, arranged residents in violation of regulations during the flood season, and failed to rectify hidden dangers.

The investigation team found that the main responsibilities of Shudao Investment Group Co., Ltd. and the enterprises involved were not implemented, the industry supervision responsibilities of relevant departments were not implemented effectively, and the territorial responsibilities of the party committees and governments of Liangshan Prefecture, Jinyang County, and Lubi Town were not implemented in place.

In accordance with regulations, the investigation team shall transfer clues and relevant materials discovered during the investigation to the Sichuan Provincial Commission for Discipline Inspection and Supervision for the investigation of accountability and accountability.

In view of the problems exposed in the course of disaster response and handling, the investigation team summed up the main lessons in five aspects: Some leading cadres do not have a sufficient understanding of the extremity of disasters and lack a strong sense of risk and bottom-line thinking; they do not have a strong party spirit and principles, lie about and conceal the number of missing people, and touch the "red line" of law and discipline; they do not carry out their responsibilities for flood prevention and do not implement them in place; they do not have a sound mechanism for transferring and avoiding risks, and they do not implement the "call and response" in place; and there are loopholes in daily supervision and management, and hidden dangers of problems have existed for a long time.

The investigation team put forward five suggestions for prevention and improvement measures, emphasizing the need to earnestly improve the political position and firmly establish the concept of safe development, refine the implementation of flood prevention responsibilities and strengthen flood prevention safety supervision, earnestly strengthen source control and comprehensively investigate and rectify hidden dangers, strengthen the management of emergency plans, improve the early warning and response mechanism, do a good job in publicity, education and training, and improve disaster prevention and mitigation capabilities.
