On the evening of December 25, 2023, the Emergency Trauma Center of the First Hospital of Zhejiang University received a family from Momo (pseudonym) who lived in the west of Hangzhou. On the same day, the 5-year-old Momo family of three wanted to make tea around the stove on a whim, so they set up a shelf on the iron charcoal stove, and while boiling tea, they roasted chestnuts, sweet potatoes, peanuts, and longan by the pot. The weather was cold, the stove was warm and the tea was fragrant, and Mom Momo also deliberately closed all the doors and windows of the big house, wanting to make the family members who did not wear coats in the house warmer.

5-year-old Mo Mo had been sitting by the stove playing with his tablet, but suddenly fainted and fell to the ground and convulsed. Parents also experience symptoms such as dizziness, headache, tinnitus, dizziness, nausea and vomiting.

The male owner with mild symptoms staggered to open all the doors and windows in the house, and silently transferred to a ventilated place, and the child woke up on his own after about half a minute, but he also had symptoms such as nausea and vomiting. They quickly dialed the 120 emergency number and were rushed to the first phase of the headquarters of the First Hospital of Zhejiang University.

The medical staff of the emergency trauma center and the hyperbaric oxygen chamber raced against time to carry out hyperbaric oxygen therapy on the poisoned Mo Mo and his family. Because of the short time of poisoning and the timely treatment of the doctor, several people have been stabilized, but carbon monoxide poisoning has the risk of "delayed encephalopathy", and if there is not enough hyperbaric oxygen therapy in the follow-up, it may leave sequelae, and they will follow the doctor's instructions for follow-up treatment.

While the family of three was still being treated, 73-year-old Grandma Li (pseudonym) from Liangzhu, Yuhang District, Hangzhou, was also sent to the rescue, and her condition was even more critical. On December 26th, the family gathered in Grandma Li's house to drink tea and dispersed, Grandma Li added a few pieces of charcoal fire thinking that she could keep warm while it was hot, so she sealed the stove, closed the doors and windows and fell asleep, who knew that the next morning when the family came to see her, found that the old man was unconscious, not only vomit next to the bed, but also incontinence. After being sent to the First Hospital of Zhejiang University for rescue, the old man was diagnosed with "carbon monoxide toxic encephalopathy".

On December 31, the New Year's Day holiday, two families had a family gathering, and a total of 9 people made tea around the stove and were sent to the first phase of the emergency trauma center of the headquarters for carbon monoxide poisoning...... Heading into the New Year, the number of poisonings continues to rise.

Some poisoned patients will seriously damage the brain after "false recovery".

"Don't underestimate the harm of carbon monoxide poisoning! We remind us every year, and some people still send them for rescue every year, and serious cases will be fatal, which is really distressing!" Yang Xiaofeng, chief physician of the director of the Emergency Trauma Center, said that compared with the poisoning caused by charcoal heating in the past, the number of poisoning caused by boiling tea around the stove this year has reached a record high, and from December 16, 2023 to January 1, 2024, 47 people who were "knocked down" by carbon monoxide were sent to a hospital in the first phase of the headquarters. The second is carbon monoxide poisoning caused by charcoal heating, self-service barbecue, etc.

Carbon monoxide is a colorless, odorless, non-irritating, lighter than air, and insoluble in water. When the carbon monoxide content reaches 0.04%~0.06%, it can poison people. To use a layman's analogy, burning a charcoal fire in a 60-square-meter enclosed room can cause poisoning in just a few minutes.

Director Yang Xiaofeng emphasized that carbon monoxide poisoning has a toxic effect on tissue cells throughout the body, and has great damage to the lungs, heart, brain and other organs of the human body, especially the cerebral cortex. In severe cases, consciousness is impaired, coma, and may even be life-threatening.

"It is especially worth emphasizing that carbon monoxide poisoning patients are not fine after waking up!" said Director Yang Xiaofeng, who said that if the poisoned patients are rescued in time, the vast majority of them can return to normal. However, after some severely poisoned patients return to consciousness, after a period of "false recovery", they may also experience a series of neuropsychiatric symptoms, such as daze, severe memory loss, unprovoked giggling or abnormal behavior, which is called "late-onset encephalopathy". Therefore, after poisoning, it is necessary to undergo systematic evaluation and adequate treatment in the hospital to avoid irreversible brain damage.

The degree of CO poisoning is graded

Mild poisoning: severe headache, dizziness, insomnia, palpitations, nausea, vomiting, malaise, blurred vision, deafness, tinnitus, mild impaired consciousness, tachycardia.

Moderate poisoning: In addition to mild poisoning symptoms, lips, nails, skin and mucous membranes appear cherry red, excessive sweating, blood pressure rises first and then decreases, arrhythmia, irritability, temporary sexual sensation and motor separation.

Severe poisoning: symptoms continue to worsen, and drowsiness, light to moderate coma, or even deep coma may occur.

Scientific prevention is even more important

Zhang Yefei, deputy chief physician of the emergency trauma center, who participated in the rescue of many patients with carbon monoxide poisoning, said that the main tool for the treatment of acute carbon monoxide poisoning is hyperbaric oxygen, which has the best effect within 6 hours of onset.

Hyperbaric oxygen therapy refers to the inhalation of pure oxygen in a high-pressure environment, and the use of more oxygen molecules to "snatch" the hemoglobin in the sick carboxyhemoglobin to re-form normal oxygenated hemoglobin and re-transport oxygen to the whole body, thereby correcting the state of cellular hypoxia.

"For acute harmful gas poisoning, early intervention of hyperbaric oxygen therapy is of great significance, which can prevent delayed encephalopathy and sequelae. Director Yang Xiaofeng emphasized that the ability of oxygen to penetrate into the local hypoxic brain tissue is enhanced, especially the hypoxic area around the hemorrhagic foci or infarct area, which improves the oxygen supply of local brain cells, restores the normal metabolism of dying brain cells, protects brain cells, increases blood flow at the ischemic site, reduces intracranial pressure, helps the recovery of consciousness disorders, and more importantly, promotes the recovery of neurological function and reduces sequelae.

In order to prevent carbon monoxide poisoning, attention should be paid to:

1. It is advisable to choose electric stoves and alcohol stoves as tea stoves indoors (the same for eating hot pot and barbecue); if you choose charcoal stoves and coal stoves, you must not use them in a closed room for a long time, and they should be placed in a ventilated hall or outdoors, and the coal should be burned out, and the charcoal fire should be completely extinguished when leaving.

2. When someone has symptoms such as headache, dizziness, tinnitus, dizziness, nausea, vomiting, etc., they should immediately think of the possibility of poisoning, and ventilate quickly, if there are moderate to severe symptoms such as confusion and coma, they should immediately call the 120 emergency number and rush to the nearest hospital for hyperbaric oxygen therapy as soon as possible.
