General situation of the building that caught fire
The building that started the fire (Jinsheng Department Store Central Store) is located at No. 2 and No. 4 Jianning Road, Gulou District, Nanjing, which is an irregular cuboid, running north-south, about 148 meters long from north to south, about 78 meters long from east to west, and about 23.6 meters high, which is a mixed structure composed of reinforced concrete structure and steel structure. The building has one basement and five floors above ground (there is a mezzanine between the fourth and fifth floors, hereinafter referred to as the four mezzanine), and the top of the fifth floor is set up with office space. The northern, east and south sides of the building are decorated with aluminum plates, and the southeast side is decorated with glass curtain walls from the second to the fifth floors. The southeast area of the fourth, fourth and fifth floors of the building stores a large number of clothing, underwear and other knitted products.
Building function: the basement floor is a garage, the first floor is department stores and catering, the second floor is department stores, catering and KTV dance halls, the third floor is home furnishing, wedding supplies, clothing and entertainment, the fourth floor is handicrafts, wedding supplies and e-commerce warehouse, the fourth mezzanine is an e-commerce warehouse, and the fifth floor is wedding supplies, training and fitness.
How the accident happened
At about 9:50 on October 29, 2022, Luo Moumou, the owner of the "Sichuan Sauerkraut Fish" shop, used an induction cooker to make breakfast for himself in the kitchen of the store.
At about 10:08, Luo Moumou had breakfast at the seat near his store in the dine-in area.
At about 10:10, the owner of the shop adjacent to the "Yunnan Mixian" on the north side found that there was a fire in the "Sichuan Sauerkraut Fish" store and notified Luo Moumou, who returned to the store and found that there was an open flame on the kitchen stove and on the surface of the range hood.
At 10:12, the 119 command center of the Nanjing Fire and Rescue Detachment received a call from the public for the first time.
At about 10:13, Wang Mouyi, a store security guard, found smoke at the end of the south side of the aisle on the fourth mezzanine above the food city. At the same time, there was a fire above the ceiling of the "Sichuan Sauerkraut Fish" shop, and smoke came out of the horizontal main oil smoke pipe above the ceiling of the shops on the south side of the store and the shops such as "Xinjiang Fried Rice Noodles", "Duoliduo" and "Northern Dumplings".
The initial disposal of merchants and Jinsheng Department Store central stores
After Luo Moumou learned of the fire, he immediately rushed into the store and used a fire extinguisher to dispose of the open flames on the kitchen stove and on the surface of the range hood hood, and then the owner of the "hot and sour noodles" shop brought a fire extinguisher to assist in extinguishing the fire.
After the open fire on the stove was extinguished, Luo Moumou was prompted to find that there was a fire in the smoke pipe above the ceiling of the store, and then he climbed into the ceiling access of the vacant shop adjacent to the south side with the help of a ladder and used a fire extinguisher to extinguish the fire.
Other shop owners in the food city and employees of the central store of Jinsheng Department Store used ladders to enter the top of the ceiling and use fire extinguishers and indoor fire hydrants to help extinguish the fire.
At 10:10, the host of the automatic fire alarm system of the central store of Jinsheng Department Store reminded that there was a fire alarm in the food city area, and Zhu Moujia, the officer on duty in the fire control room, notified the security guard Wang Mouyi to go to the scene of the food city and the fourth mezzanine to verify the fire.
At 10:17, security captain Wang Mou C and security guard Liu Mou B went to the food city on the second floor to participate in the fire disposal.
At 10:19, Zhu Moumou A reported the fire to the store manager Li Moumou and the property manager Zhu Moumou B by phone, and the store manager Li Moumou then arranged personnel to dispose of it.
Zhu Moujia, as the duty officer in the fire control room, did not immediately set the fire alarm linkage control switch to the automatic allowable state after confirming the fire, nor did he dial "119" to call the police. It was not until 10:45 a.m. that the host of the automatic fire alarm system was set to an automatic allowable state after consulting Wang Mou, and then went to the pump room to set the fire hydrant pump and sprinkler pump control cabinet switch from manual state to automatic state.
Cause of fire
Luo Moumou, the owner of the "Sichuan Sauerkraut Fish" shop in the food city on the second floor of the shopping mall, used the induction cooker to make breakfast for himself in the kitchen of the store, and left the kitchen without determining that the induction cooker was turned off.
The cause of the spread of the fire
The outdoor vertical fume pipe of the food city is laid close to the exterior wall on the southeast side of the building, which is not sealed and intact, and the exterior wall and the glass curtain wall form a closed space on the southeast side of the building. After a large number of high-temperature open flames and smoke in the oil smoke pipe broke through the pipe wall, they quickly passed through the gap at the top of the outer wall in the enclosed space and entered the fifth floor, fourth mezzanine and fourth floor ceiling of the shopping mall, and ignited a large number of combustible materials such as knitted clothing stored in the warehouse on the southeast side of the fourth mezzanine and the fourth floor. At the same time, the hot flue gas increases the internal temperature of the shopping mall, and a large number of solid combustible substances begin to pyrolyze, and begin to burn after reaching a certain temperature, and spread horizontally and vertically, successively causing multiple stores and warehouses with more than three floors to catch fire, showing a trend of multi-point combustion development, forming three-dimensional combustion, and local collapse. The details are as follows:
1. The setting of the oil smoke pipe is unreasonable and a large amount of oil scale accumulates, resulting in the rapid spread of the fire. The horizontal main fume pipe of the food city of Jinsheng Department Store Central Store is not sealed intact, and the vertical fume pipe is set in the closed space on the southeast side of the building, without a special shaft, and there is no fire separation between it and the outer wall of the original building. In addition, a large amount of oil scale accumulates in the fume pipe, and the risk of fire spread increases, and when a fire occurs, the fire spreads rapidly along the fume pipe due to the chimney effect and the roof range hood fan is open.
2. The enclosed space between the exterior wall and the decorative curtain wall causes smoke to spread into the building and affects the fire fighting and rescue operations. After the fire broke out in the vertical smoke pipe located in the enclosed space on the southeast side of the building, a large amount of high-temperature smoke accumulated and could not be released to the outside, and it concentrated to break through to the inside of the building. The enclosed space between the exterior wall of the building and the decorative curtain wall seriously affects the fire fighting and rescue operations.
3. The fire insulation performance of the exterior wall on the southeast side of the building is weak and there are holes and gaps, resulting in the fire breaking through quickly to the inside of the building. After sampling and testing the metal surface rock wool sandwich panel remaining on the exterior wall on the southeast side, the fire resistance time was 0.2 hours. The high temperature (or open flame) in the vertical oil smoke pipe caused by the superposition of various reasons quickly conducted (or burned through) the pipe wall and the outer wall, igniting a large number of combustibles such as knitted clothing stored in the store on the southeast side, causing the fire to burn violently and continue to expand.
4. Some fire-fighting facilities were not started in time, resulting in the initial fire not being effectively controlled. The fire control host of the central store of Jinsheng Department Store is in an automatic prohibition state, and the switch of the fire pump and sprinkler pump control cabinet is in a manual state, resulting in automatic sprinkler fire extinguishing, mechanical smoke exhaust, fire shutter and other building fire protection facilities not playing a role in time, and some items are stacked under the fire shutter and cannot be completely lowered to the ground.
5. The management requirements of the fire control room and the emergency procedures on duty are not standardized. The switch of the control cabinet of the sprinkler pump and fire pump is set in an automatic prohibition state for a long time. On the day of the fire, there was only one person on duty in the fire control room and did not obtain the professional qualification certificate for the specific type of work in the fire protection industry, and the personnel on duty did not know the emergency response procedures, did not call the alarm, and did not set the host of the automatic fire alarm system in the automatic allowed state in time, which affected the start of the fire linkage facilities.
The main problems exposed by the units involved in the accident
(1) "Kawasour Vegetable Fish" store
Luo Moumou left the kitchen without confirming that the induction cooker was turned off, and the residue in the pot continued to heat up and caught fire, causing the fire to spread along the smoke pipe.
(2) Jinsheng Real Estate
1. Failure to organize regular cleaning of the fume pipeline on the second floor of the central store of Jinsheng Department Store in accordance with relevant regulations, resulting in the accumulation of a large amount of oil in the pipeline and increasing the risk of fire spread;
2. In violation of the relevant fire protection technical standards and specifications, the fire control host and fire pump and sprinkler pump control cabinet of Jinsheng Department Store Central Store have been in an automatic prohibition state for a long time, resulting in the fire shutter, automatic sprinkler fire extinguishing, mechanical smoke exhaust and other building fire protection facilities of the shopping mall have not played a role in time, and the daily management is not good, and some of the items stacked under the fire shutter have not been completely lowered to the ground.
3. Failure to provide the necessary funds for the fire safety of the central store of Jinsheng Department Store in accordance with the regulations, only the maintenance of some building fire protection facilities (automatic fire alarm system, fire hydrant fire extinguishing system, smoke prevention and exhaust system and fire shutter door), and the annual inspection of building fire protection facilities.
4. Failure to take timely measures to eliminate fire hazards in accordance with regulations, serious luck with fire accidents that may occur in the central store of Jinsheng Department Store, and prevarication and failure to rectify fire hazards such as destroying fire prevention partitions, setting up warehouses on the roof of buildings in violation of regulations, and failing to set up fire protection facilities in the atrium found by relevant departments and units during inspections. The recurring fire hazards such as blocking fire shutters, occupying evacuation channels, blocking safety exits and blocking fire hydrants in the central store of Jinsheng Department Store are not effectively controlled.
5. The fire safety system and emergency procedures approved by the enterprise do not meet the requirements of the specification, and when the fire occurs, the fire control room duty personnel do not dispose of it in accordance with the prescribed procedures, fail to operate the fire protection facilities correctly, and do not dial the "119" fire alarm telephone;
6. Transforming the original part of the shopping mall area on the second floor into a catering establishment, failing to go through the relevant procedures such as construction permits, fire protection design review, completion acceptance filing, and pre-opening fire safety inspection in accordance with regulations.
7. Expanding the scope of construction without following the planning permission of temporary construction projects, carrying out construction illegally, and refusing to perform the decisions made by relevant departments and units to stop construction and demolish within a time limit.
8. Failing to fully implement the main responsibility for fire safety in accordance with the regulations, failing to clarify the fire management organization of the headquarters and equipping special fire safety management personnel in combination with its own business characteristics and management mode, and failing to supervise the implementation of the relevant fire safety main responsibilities of the First Construction Group and Jiangsu Zhendong in accordance with the regulations.
(3) Zhendong, Jiangsu
Jiangsu Zhendong, as the lessee of the building and the signatory of the fire protection and maintenance agreement, failed to implement its own fire safety responsibilities, failed to supervise the contracted fire protection and maintenance unit to perform the content of the contract, and the legal representative was not in place to perform his duties for a long time.
(4) Yijian Group
1. As the property right unit of the legal building of the central store of Jinsheng Department Store, the fire safety responsibilities of all parties were not clarified in accordance with the regulations, and the fire safety responsibilities of the unit were not performed effectively.
2. As the foundation and basement construction unit of the reconstruction and expansion project, the construction project violates the regulations and does not obtain the construction permit without authorization.
(5) Nanjing Jinchen
As the maintenance unit of the fire protection facilities of the building where the incident occurred, it failed to carry out fire protection technical service activities in accordance with the standards, and failed to maintain the system equipment and components within the scope of maintenance according to the requirements of the contract.
(6) Jiangsu Changhong
As the steel structure construction unit of the reconstruction and expansion project, it carried out the construction without obtaining a construction permit.
(7) Gangning, Jiangsu
As the interior decoration construction unit of the reconstruction and expansion project, the construction was carried out without obtaining a construction permit.
(8) Nanjing Fire Company
As a fire protection construction unit of the reconstruction and expansion project, it carried out construction without obtaining a construction permit.
The main problems of the relevant departments and territorial governments
(1) Fire department
1. District fire department
The fire inspection work carried out by the former Xiaguan District Public Security Fire Brigade was not standardized, and the reconstruction project of the food city on the second floor of the central store of Jinsheng Department Store was unclear, resulting in it not being included in the fire supervision for a long time. In the daily fire supervision and inspection, the Gulou District Fire and Rescue Brigade failed to timely discover the problems of insufficient number of personnel on duty in the fire control room of the central store of Jinsheng Department Store, the lack of certificates, and the failure to regularly clean the oil smoke pipes.
2. Municipal fire department
The Municipal Fire and Rescue Detachment failed to effectively supervise and guide the Gulou District Fire and Rescue Brigade to carry out fire supervision and inspection.
(2) Territorial governments
1. Jianning Road Street, Gulou District
Since 2019, no effective measures have been taken to rectify fire hazards such as occupying evacuation channels and blocking safety exits in the central store of Jinsheng Department Store.
2. Gulou District Government
The Gulou District Government failed to implement its territorial management responsibilities and failed to effectively supervise and guide relevant departments to implement fire safety and safety production supervision responsibilities.
3. Former Shimonoseki District Government
The former Xiaguan District Government did not organize relevant departments to take effective measures to stop and investigate the illegal construction of the central store of Jinsheng Department Store.
(3) Department of Commerce
1. Gulou District Bureau of Commerce
He did not conduct in-depth research on how to fulfill his fire safety responsibilities, and did not take the initiative to have an in-depth understanding of the fire safety situation of the central store of Jinsheng Department Store from the district fire rescue agency and Jinsheng Real Estate; did not take effective measures to guide and supervise the rectification of the hidden safety hazards of the central store of Jinsheng Department Store assigned by the Municipal Bureau of Commerce; the joint inspection was a mere formality, with many inspections and few problems found.
2. Municipal Bureau of Commerce
The fire safety management of the commercial and trade industry is not in place, and the follow-up and guidance on the implementation of the main responsibility of the central store of Jinsheng Department Store by the Gulou District Bureau of Commerce is not in-depth.
(4) Nanjing Decoration Industry Development Center (formerly Nanjing Decoration Industry Management Office)
The supervision and management of the renovation project of the food city on the second floor of the shopping mall was not effective, and the illegal acts of unauthorized construction of the above-mentioned renovation project without obtaining a construction permit in accordance with the regulations were not discovered and investigated in a timely manner.
(5) Gulou District Emergency Management Bureau
During the special rectification period of production safety, the supervision and implementation of the "retrospective" work of rectifying safety issues was not thorough.
According to the report, Luo Moumou, the owner of the "Sichuan Sauerkraut Fish", and Li Moumou, the manager of the central store of Jinsheng Department Store, were investigated by the public security organs on suspicion of the crime of fire and major liability accidents, and two of them have been transferred for prosecution.
In the investigation of the cause of the accident and the extended investigation of daily management, 13 party members and cadres of fire and rescue agencies, Gulou District Government, Jianning Road Street, commerce, urban and rural construction, emergency management and other departments were found to be derelict in their duties and responsibilities, and 1 party member cadre was held accountable, and the Gulou District Government, the Municipal Fire and Rescue Detachment, and the Municipal Bureau of Commerce were instructed to make a deep inspection to the municipal government.
