Since the beginning of this year, the Boli County Medical Security Bureau has taken improving people's livelihood and uniting people's hearts as the starting point and foothold of its work, and has taken advantage of the opportunity of medical insurance to promote the pilot work of building a primary medical and health service system, and accurately adopted "six in place" measures to help rural revitalization and improve people's livelihood.
Debriefing communication is in place
Since the Provincial Medical Security Bureau determined that Qitaihe City would be the only pilot city in the province to promote the construction of the primary medical and health service system, the Boli County Medical Security Bureau actively coordinated and communicated with the Municipal Medical Security Bureau in combination with the actual development of the county's primary medical and health institutions, and explored the feasibility of including some township health centers in the county into the pilot unit. After the Municipal Medical Security Bureau issued the Notice on Printing and Distributing the Implementation Plan for the Pilot Work of Promoting the Construction of Primary Medical and Health Service System in Qitaihe City Medical Insurance (Trial), it once again actively fought for and successfully included Xiaowuzhan Town and Shuanghe Town Health Center into the first batch of pilot units in our city.
Strengthen leadership and coordination in place
After the kick-off meeting of the pilot work of promoting the construction of the primary medical and health service system in the city, Boli County immediately formulated the "Implementation Plan for the Pilot Work of Promoting the Construction of Primary Medical and Health Service System in Boli County", which clarified the work objectives and work content, detailed the implementation steps and relevant requirements, and strengthened the organization and coordination, publicity and supervision. A leading group for pilot work has been set up, and the office is set up in the county medical security bureau, which is responsible for coordinating the arrangement, deployment, organization and implementation, assessment and evaluation of the pilot work, summary and promotion. The task division of the leading group is clear, the responsibility is compacted in place, and the strict control and joint efforts are made to ensure that the pilot tasks are compacted and implemented.
Grassroots work guidance is in place
From June 8 to 10, the medical security bureaus at the city and county levels respectively carried out on-site visits and investigations to 10 township health centers in the county to understand in detail the basic and service conditions of township health centers. On June 16, the medical security bureaus at the city and county levels once again went to Xiaowuzhan Town and Shuanghe Town Health Centers to conduct field inspections and deploy pilot related work. The two units are required to strengthen the incentive management of grassroots medical and health service personnel, change traditional passive services to active services, and guide medical personnel to carry out activities such as itinerant medical treatment, health management, family beds, and door-to-door services in the form of contracts, so as to provide effective health services for villagers.
All businesses are integrated into place
Combined with the current situation of the shortage of grassroots medical resources in the county, the pilot work leading group actively coordinated with organizations, human resources and social security departments and other departments, improved the working mechanism for the introduction of grassroots medical talents, and actively attracted college graduates to work in primary medical institutions. At the same time, the county health department has carried out standardized training and transfer training for county general practitioners, and further optimized the general practice concept, clinical diagnosis and treatment services and medical insurance business service capabilities of professional doctors. On-the-job vocational training has been carried out for family-contracted doctors, focusing on targeted and operable applicable skills, effectively improving the ability of family-contracted doctors to diagnose and treat chronic, common and frequent diseases, and the reimbursement and service capabilities of outpatient chronic diseases and outpatient "two diseases".
Industry policies are in place
Actively give play to the role of family doctors in guiding hierarchical diagnosis and treatment, and promote cooperation among medical alliances. The county people's hospital and the traditional Chinese medicine hospital signed medical community agreements with Xiaowuzhan Town and Shuanghe Town respectively to carry out all-round medical cooperation, and refer patients beyond the diagnosis and treatment capacity of township medical institutions to support hospitals for treatment. Support hospitals to set up green channels for referral, and then transfer patients back to townships for follow-up treatment after their condition is stable, so as to achieve two-way referral and implement graded diagnosis and treatment. At the same time, establish a drug demand account, make an appointment for the personalized drug needs put forward by patients, and equip relevant drugs as soon as possible when the requirements are met.
Incentives are in place
Accurately give full play to the leverage of medical insurance payment, implement family doctor contract medical insurance services, and scientifically determine that family doctor contracts include general practice appointment treatment, appointment referral, long prescription service, outpatient coordination, chronic special diseases, "two diseases" guidance medication, home service, patrol service, etc. At the same time, establish an assessment system of "basic services + incentive rewards", guide villagers to adopt the form of "contractual" medical services, and initially establish the "Boli County Family Doctor Contract Service Medical Insurance Incentive Assessment Index System" on the basis of the payment management of medical insurance funds for basic medical insurance services contracted by family doctors, and evaluate from eight indicators such as the growth rate of per capita hospitalization cost of the contracted objects, so as to promote the improvement of the overall service efficiency of family doctor contracting. At the same time, a veto target is set up, and the incentive assessment qualifications are disqualified for low satisfaction of the contracted objects and the occurrence of fraudulent medical insurance funds.
The "six in place" measures have accurately promoted the steady implementation of the construction of the primary medical and health service system. As of August 10, Xiaowuzhan Town Health Center has established 6 family doctor teams, with 11,192 people contracted, 21 referrals, 239 matching drug types, and 1,619 door-to-door services. Shuanghe Town Health Center has established 19 family doctor teams, with 11,107 people contracted, 10 referrals, 187 matching drug types, and 4,323 door-to-door services.
