For anesthesia, due to the thick back fat, it is difficult to reach the intervertebral space with commonly used anesthesia needles, and it is difficult to place the low head and knee arch back position of normal anesthesia, and anesthesia is extremely difficult. The thick fat in the abdomen of pregnant women narrows the visible range and operating space of the abdominal cavity, which greatly increases the difficulty of surgery and the risk of infant retrieval. With rich experience, Director Xu Fudong of the Department of Anesthesiology directly used a lumbar anesthesia needle to successfully wear it once, showing the "needle" skill. Wu is 32 years old, a new mother, weighing 240 pounds, and her pregnancy is 37+2 weeks. Unlike Pang, Wu is not only heavy, but also has chronic hypertension combined with pregnancy, and diabetes during pregnancy. Blood pressure during pregnancy is up to 190/110mmHg, and hospital blood pressure is 160/110mmHg. Although there may be some risks, after evaluation of pelvic conditions and fetal size, it is suitable for vaginal delivery, and the patient himself insists on a vaginal delivery. After completing the relevant examinations and having no other abnormalities, after consultation with Director Zhang Li, it was decided to be admitted to the hospital and induced labor. After Wu was admitted to the hospital, he was given antihypertensive therapy to stabilize blood pressure, monitor the waiting for delivery, and closely observe the labor process and various vital signs.
At 18:15 on April 4, Wu entered the delivery room, and Director Wang Linqing and Liu Xiaoxia, the head nurse, guarded the whole process, deployed personnel in the department, and were ready to deal with various emergencies at any time. Due to Wu's persistent occipital transverse position, active period stagnation during labor, abnormal fetal orientation, and ineffective medical intervention, continued delivery may cause fetal intrauterine distress at any time, endangering fetal life. Director Wang Linqing immediately decided to terminate the pregnancy for emergency cesarean section, and personally took charge of the operation, successfully delivered a 5.5-pound healthy baby girl, and the mother and daughter were safe.
