Prevent birth defects and promote reproductive health. September 12 every year is China's Birth Defect Prevention Day.

With the country's increasing attention to the prevention and control of birth defects, in the development process of guiding the whole society to comprehensively prevent and treat birth defects and improve the service level of eugenics and breeding, the strengthening of the construction of the talent team is indispensable.

In 2020, the Ministry of Human Resources and Social Security, the State Administration of Market Regulation and the National Bureau of Statistics jointly released 16 new occupations to the society, among which birth defect prevention and control consultants were listed, and were known as the "quality inspectors of destiny" for newborns at the beginning of life.

Prevention and control of birth defects

The total incidence of birth defects in mainland China is about 5.6%

Prevent and reduce birth defects, put good life health first. As early as 2016, the Party Central Committee and the State Council issued the "Healthy China 2030" Planning Outline, which clearly pointed out that it is necessary to strengthen the comprehensive prevention and treatment of birth defects, and build a birth defect prevention and control system covering urban and rural residents, covering all stages of pregnancy, pregnancy and newborns.

What is a birth defect? It is understood that it refers to the abnormalities of body structure, function or metabolism that occur before the birth of the baby, which is caused by environmental factors, genetic factors and the interaction of the two, and is the main cause of early miscarriage, stillbirth, infant death and congenital disability.

The Ministry of Human Resources and Social Security issued the "New Occupation - Analysis Report on the Employment Prosperity of Birth Defect Prevention and Control Consultants", which mentions that there are more than 8,000 known birth defects, genetic factors such as genetic mutations and environmental factors can cause birth defects.

It is estimated that the overall incidence of birth defects in the mainland is about 5.6%. According to the data released by the "China Birth Defect Prevention and Control Report (2012)", the number of new birth defects in China is about 900,000 every year, of which about 200,000 cases of birth defects are obviously clinically visible at birth.

Yang Fang, director of the Department of Fetal Medicine and Prenatal Diagnosis of Zhujiang Hospital of Southern Medical University, has been engaged in prenatal diagnosis for 15 years, which can be described as witnessing the development process of the continuous construction of national birth defect prevention and control. She told reporters that Guangdong Province is the province with the largest number of prenatal diagnostic centers in China, accounting for one-eighth of the number of prenatal diagnostic centers in the country. The awareness of birth defect prevention and control in grass-roots hospitals and ordinary people is still not high, and in the final analysis, there is still a lack of professional and high-level birth defect analysis and genetic counseling talents.

In fact, the state has always regarded the improvement of the quality of the population as the primary task of population policy. In Yang Fang's view, how to "give birth to a good one" under the existing population conditions has a lot to do with the screening of birth defects.

At present, the medical level of resources in all regions of the country is uneven, many grass-roots hospitals do not have pediatric surgery, the understanding of birth defects is not deep enough, either failed to find the problem at the first time, or found that the fetus has an abnormal condition may recommend pregnant women to induce labor, but many times these abnormalities can be treated by intrauterine treatment or neonatal surgery.

"Because of the lack of awareness of the disease, the child is induced into labor before he can see the world, which is contrary to our country's concept of advocating the prevention and control of birth defects."

There is no doubt that birth defects not only bring great pain and economic burden to families and society, but also open the door to ill-fatedness for newborns themselves. The birth defect prevention and control consultant who plays the role of the frontier "gatekeeper" came into being on the basis of the vigorous construction of the national birth defect prevention and control system, and became the "destiny quality inspector" of the starting point of the newborn's life.

New career and new atmosphere

Birth defect prevention and control counseling is more standardized and professional

On February 25, 2020, the Ministry of Human Resources and Social Security, together with the State Administration of Market Regulation and the National Bureau of Statistics, jointly released 16 new occupations to the society, and birth defect prevention and control consultants are one of them.

"We are all optimistic about the prospects for new careers." Yang Fang said: "For the introduction of this new profession, the department was very enthusiastic at that time, and 2 people signed up for the training of birth defect prevention and control consultants in the province just enrolled. ”

Wang Jian, a "post-90s" technician, is one of the participants in the training. After graduating, he came to the Prenatal Diagnosis Department of Pearl River Hospital and was responsible for writing diagnostic reports. During the two years he worked in the hospital, he knew that his understanding of birth defects was still very superficial, and "many predecessors have been constantly expanding the content and scope of knowledge."

In the process of actively learning prenatal diagnosis, birth defects and other related knowledge, the birth defect consultant training course attracted Wang Jian's attention. "The course probably covers the three levels of prevention and control, as well as the key points of common genetic diseases and diagnostic consultation, and the lecturers are also experts in the industry, and the quality of the lectures is high." This is an opportunity for me to systematically replenish my knowledge. He said that the results of systematic learning in this half year are still quite remarkable, and "the knowledge learned in the university is broken and reorganized." "For example, the course will talk about the knowledge of the ultrasonic standard aspect, which has a certain effect on the daily work now."

Most of the people who participated in the birth defect prevention and control consultant course with Wang Jian at the same time were technicians and nurses from major hospitals, and some were related personnel of medical companies engaged in birth defect testing, and the population was widely distributed.

In the course of training, he also gradually gained a deeper understanding of the new profession of birth defect prevention and control consultant. "If you can find abnormalities before the birth of a child and solve them in time, it will be good for both families and individuals, and it will also reduce the burden on society."

In his view, the release of new occupations has indeed raised birth defect prevention and control counseling to a more standardized and professional level, which not only provides new career choices for practitioners, but also provides new development directions and promotion space for experienced practitioners. Wang Jian told reporters: "In fact, this is also to promote the work of three-level prevention and control of birth defects and promote the construction of a fertility-friendly society in Guangdong." ”

The talent gap is large

Experts are calling for more young people to join

After the release of the new occupation, Guangdong Province also established the Birth Defects and Prevention and Control Committee of the Preventive Medicine Association. Since its establishment, the committee has held academic seminars and science popularization activities to do a good job in the prevention and control of birth defects, and popularize the awareness of birth defect prevention and control to thousands of households.

In August this year, the second annual meeting of the Birth Defect Prevention and Control Professional Committee of the Guangdong Preventive Medicine Association and the third Pearl River Forum on Obstetrics and Gynecology Ultrasound and Fetal Medicine were held in Guangzhou, focusing on the theme of "birth defect prevention and control" and other topics. At the same time, as the organizer of The Pearl River Hospital of Southern Medical University, its 800-square-meter new area of fetal medicine and prenatal diagnosis was officially launched.

"Of course, I hope that after the release of the new career, the relevant counseling will be more popular and professional." However, in the process of building a fertility-friendly society, the people's awareness of the prevention and control of birth defects is also indispensable. Yang Fang said that allowing young people to attach importance to prevention and control measures such as marriage examination and obstetric examination has a lot to do with the deepening of publicity and education in the whole society.

The reporter learned that about 15 million newborns are born on the mainland every year, and about 12.79 million couples are planned to become pregnant. According to the family's consultation needs, government agencies at all levels, maternity hospitals, children's hospitals, professional public health and other institutions at all levels should be equipped with at least 1-2 birth defect prevention and control consultants. The "New Occupation - Analysis Report on the Employment Situation of Birth Defect Prevention and Control Consultants" pointed out that the demand for birth defect prevention and control consultants is expected to be about 1 million in the next 5 years.

In this regard, Yang Fang very much hopes that more young relevant practitioners, such as technicians, nurses, clerks, grass-roots medical staff, etc., can join the training team of birth defect prevention and control consultants. "Many healthcare professionals come into contact with patients earlier than our clinicians, for example, many pregnant women call for counseling. If the medical staff who first came into contact with patients can have the knowledge of birth defect prevention and control, or become birth defect prevention and control consultants, then they can do a good job of consultation, help patients understand the importance of birth defect prevention and control, and go to professional institutions for diagnosis and treatment as soon as possible. ”
