The 13th People's Hospital of Chongqing (Chongqing Geriatric Hospital) has always adhered to the concept of providing better medical services and health management services for the masses and elderly patients, constantly explored new models of smart health services, and successfully created two wisdom crystals of "health hut" and "blood glucose whole tube", with information and intelligent means, to achieve timely and accurate health intervention for patients, effectively enhance patients' sense of medical access and satisfaction, and comprehensively protect the lives and health of the masses.
Intelligent "health hut" to create a high-quality intervention center
On December 26, 2018, the "Health Hut" of Kowloon Bay Community built by the hospital and the Kowloon Bay Community Neighborhood Committee of Huangjueping Street was officially launched, and through providing free physical examination measurement, health education and health intervention guidance for community residents, the elderly home health management system was initially established, which effectively enhanced the sense of medical service access of the people in the jurisdiction.
According to the staff of the "Health Hut", after the residents in the "Health Hut" carry out physical examination items such as height, weight, heart rate (pulse rate), ECG, basal metabolism, random and fasting blood glucose, blood oxygen saturation, etc., the intelligent physical examination equipment will synchronously store and analyze the examination results, transmit the physical examination report and evaluation report to me through WeChat, and at the same time transmit it to the Thirteenth People's Hospital of Chongqing Through the mobile medical information system, giving full play to the advantages of the professional medical team of the secondary hospital. The use of data analysis to provide health guidance and intervention to residents also facilitates the doctor's access to historical data when residents visit doctors later.
"Since I have the 'Health Hut', I have been able to do routine examinations at my doorstep, and through a unified data platform, doctors can understand my physical condition in the recent period and achieve more accurate and effective diagnosis and treatment." Up to now, there are 2203 households in the Kowloon Bay community, the staff has visited the door 4422 times, opened 1285 households, established 297 households for physical examination, 683 people have filed physical examinations, 2017 people have been physically examined, and 15433 health data have been collected. Organized 15 community training knowledge lectures, organized 12 community free clinic publicity activities, and received 16 visits from groups and units.
In order to build a more perfect health management system, in the first half of this year, the hospital has reached a tripartite cooperation agreement with Ali Bian Que and Chongqing Xinlongxin Property to start the construction of the second batch of "health huts", that is, in the Loncin International Community, to explore the establishment of a "elderly home health management intervention system based on the Internet + community + medical institution model".
The hospital invested in the second batch of "health hut" pilot innovation, through the establishment of community health management service stations, continuous collection of public health signs data, and the establishment of dynamic health files for the public. In addition, the resident health management intervention center is established within the hospital, and the general medicine (science) treatment department is set up, and relying on it, all kinds of common chronic diseases (hypertension, diabetes, coronary heart disease, etc.), psychiatry, psychology, sleep, pediatrics, general medicine and other professional medical personnel are organized to create a high-quality resident health management intervention center.
After the upgrade, the second batch of "health huts" will build an Internet + medical public health service center platform to provide personal chronic disease management, cardio-brain detection, health consultation, real-time online detection, emergency rescue, follow-up of home patients, drug distribution, off-card payment, remote online and offline follow-up, electronic prescription, personal disease prediction, AI intelligent Q&A and other services for the elderly at home; integrate the service system centered on regional public hospitals and assisted by property services, empowered by Internet tools. Through the service communication system of family health housekeepers (community health hut + property golden housekeeper), it provides better and more convenient smart health management services for residents in the jurisdiction.
Innovative medical model, launch of digital "blood glucose whole tube"
On April 15 this year, the hospital took "chronic disease management" as a breakthrough and launched the "blood glucose whole process management" project, taking the lead in the country to open an innovative attempt to open the remote out-of-hospital management model of diabetes, reshape the management mode of diabetes, and realize smart health services for diabetic patients.
This is undoubtedly a good thing for Zhang Yingmei, a diabetic who lives in Qijiang District. "My blood sugar is often high, and I have to come to the hospital every three to five minutes for examination, which is really inconvenient!" Since the implementation of the "blood glucose whole tube", Zhang Yingmei can monitor blood glucose without leaving home and enjoy the health guidance of the professional medical team.
Through the digital diabetes management system of "Ruicon Sugar", the "Whole Blood Glucose Management" project combines traditional diabetes treatment (in-hospital inpatient treatment) with out-of-hospital treatment after discharge, forming a virtuous cycle of full management of blood glucose control.
At home, patients can use the blood glucose meter to collect blood glucose data by themselves, and by uploading blood glucose data, a powerful artificial intelligence system can help improve the patient's health management ability. "I usually use the system to set a time to remind myself to monitor my blood sugar and medication, and to check my blood sugar overview at any time." Since using the digital system to manage blood sugar, Zhang Yingmei has reduced the number of times she runs to the hospital, developed blood glucose monitoring habits, and enhanced her awareness and ability to self-manage diabetes.

The diabetes management big data cloud platform allows patient data to be interconnected with medical institutions in real time. Sun Wenjing, deputy chief physician of the Department of Gastroenterology and Endocrinology, said that medical staff only need a computer or tablet to log in to their account to see patient data in real time, and the multi-dimensional professional report in the background allows doctors to quickly understand the patient's situation at a glance. The system will also issue alerts when high and low blood glucose crisis values occur, reminding doctors to give patients timely health interventions when necessary, telephone follow-up, medication advice, and diet and exercise recommendations.
The "Whole Blood Glucose Management" project is the first intelligent people's livelihood project in Southwest China where medical institutions apply advanced information technology platforms to manage diabetic populations. The project model expands the time and space of medical services, and truly realizes the medical innovation model of "letting data run more and the masses run less errands".
After several months of management, the project achieved good results, with 164 diabetic patients included in the system management, of which 65.9% of the patients (108) received more accurate diagnosis and treatment of drug regimens.
In addition, the hospital is carrying out the construction of the "medical, nursing, health and nursing" system, integrating and upgrading the 11 systems that have been operated, forming a platform construction, integrating the "islanded" and "fragmented" information, and realizing the interconnection and data sharing between the systems, so as to better provide patients with high-quality and convenient medical services.
